Rehabilitation

Hamilton Continuing Care Center offers a separate Short-Term Rehabilitation Unit for patients who need additional support and recovery following a hospital stay. We begin planning for discharge on the day the patient arrives. The Rehabilitation professionals are members of the interdisciplinary team that work together with admissions, nursing, social services, dietary, activities and all medical departments to improve the functional independence, safety and quality of life of our patients.
The goal of the Rehabilitation Department is to assist residents in achieving their maximum levels of function, self care and mobility through exercise, training and caregiver education.

Rehabilitation Services
Our Rehabilitation Services address diverse conditions resulting from debilitating illnesses and injuries that may be a result of:
- Neurological conditions, such as stroke, Multiple Sclerosis, and Parkinson’s Disease
- Orthopedic injuries and fractures
- Joint replacements
- Cancer-related conditions
- Amputations
- Trauma resulting from accident or illness
- Muscular/skeletal problems
- Functional decline secondary to other illnesses
Rehabilitation Team consists of three distinct therapy disciplines:
- Physical Therapy
- Occupational Therapy
- Speech Therapy
Each discipline focuses on a different aspect of the rehabilitation process and provides exercises and specific modalities to assist patients in achieving their rehab goals.

Physiatrist Services
Along with our highly trained Medical Care Staff, Hamilton Continuing Care Center offers the unique services of an onsite Physiatrist (a physician specializing in rehabilitation medicine). All therapies are under the supervision of the Physiatrist and patients are monitored weekly. The Physiatrist participates in the care planning and discharge planning process of each rehabilitation patient.
Outpatient rehabilitation services are also available to offer a continuum of care to all our patients and the community. Hamilton Continuing Care Center provides comprehensive outpatient Physical, Occupational and Speech Therapy services to patients with various needs. Our therapists ensure that each patient achieves his optimum level of functions, while still maintaining a “tender touch” in customer service and patient care.

For each patient in need of outpatient physical, occupational therapy or speech pathology services, a written plan of care will be established and periodically reviewed by a physician or by a physical therapist, occupational therapist, or speech pathologist respectively.
